Большинство полученных МКА к компонентам Kell-гликопротеина имели широкую направленность и реагировали с эритроцитами большинства встреча-ющихся фенотипов, за исключением Kell-дефицитных: Ko, McLeod и Kmod. [128, 144, 179, 202, 302–304, 329].

Jaber и соавт. [211] получили мышиные МКА к K-гликопротеину с мол. мас-сой 93 кДа, выделенному с помощью электрофореза в полиакриламидном геле из эритроцитов человека K +k − и K −k +. Из эритроцитов Kо и McLeod ком-понент 93 кДа авторам выделить не удалось. Антитела не агглютинировали

нативные и энзимированные эритроциты и, по-видимому, были направлены к цитоплазматическим доменам K-гликопротеина. Мембранные протеины, выде-ленные из человеческих тромбоцитов, лимфоцитов, гранулоцитов, моноцитов,

 

в также эритроцитов животных, давали такие же результаты в методе с иммуно-окрашиванием, однако при сравнительном анализе в иммуноблоте обнаружено, что эти антитела реагируют с другими эпитопами K-гликопротеина, не являю-щимися K-антигеном.

 

Jaber и соавт. [210] выделили человеческие МКА анти-K из супернатанта В-клеточной линии, трансформированной вирусом Эпштейна – Барр. Антитела реагировали с компонентом 93 кДа, присутствовавшим на эритроцитах K +, но не реагировали с эритроцитами K − и Kо, поскольку они не содержали указанно-го компонента.

Parsons и соавт. [303] получили мышиные МКА (BRIC 203) со специфично-стью анти-Kp bc, которые реагировали с эпитопами Kp b и Kp c , но не Kp a.

 

Nichols и соавт. [297] описали моноклональные антитела анти-K14 и анти-k. Анти-k-антитела получили Sonneborn и соавт. [350].

 

Tearina Chu и соавт. [367] получили МКА анти-Kp a иммунизацией мышей плазмидами, трансфектированными Kell-ДНК человека.

 

Активные МКА анти-Js b получили Chu, Yazdanbakhsh и соавт. [128] иммуни-зацией мышей клеточной линией мышиной эритромиеломы (МЭЛ), экспресси-рующей человеческий Kell-гликопротеин.

 

Некоторые МКА, идентифицированные предварительно как анти-Kell, реа-гировали с эритроцитами, обработанными АЕТ [276, 305].

 

Российской Федерации также созданы [25, 26, 31–33] и серийно произво-дятся в Гематологическом научном центре моноклональные реагенты анти-K и анти-k, которые с успехом применяют в практической работе. Разработан экспресс-метод определения фактора Kell на плоскости без подогрева с помо-щью композита, включающего поликлональные сыворотки анти-Kell и смесь конглютининов (декстрана и других коллоидных соединений) [64, 72].

 

Посредством слияния лимфоцитов периферической крови донора, ис-кусственно иммунизированного энзимированными эритроцитами K +, и клеток мышиной миеломы Х-63, нам удалось получить гетерогибридому KEL1-D / D98, продуцирующую моноклональные анти-K (МКА анти-K) IgG-антитела, которые отличались от поликлональных анти-Kell (ПКА анти-K) IgG-антител, полученных от того же иммунного донора и двух других лиц, подвергшихся естественной иммунизации антигеном K вследствие беремен-ности и гемотрансфузий.

 

МКA анти-K агглютинировали как энзимированные, так и нативные эри-троциты K + на плоскости при комнатной температуре в отличие от ПКА анти-K, которые обладали способностью агглютинировать только энзимирован-ные эритроциты K +. Другие серологические параметры ПКА анти-K и МКA анти-K практически не отличались: они имели одинаковый титр – 1 : 32 ± 64

С непрямой пробе Кумбса, слабо реагировали в реакции конглютинации в про-бирках с желатином, но в то же время выраженно агглютинировали нативные эритроциты K + на плоскости при комнатной температуре в комбинации с кон-глютининами декстранового ряда.

 

Полученные данные свидетельствуют о своеобразии различий ПКА анти-K и МКA анти-K, а также о полиморфизме внутри такой серологически четко очерченной антигенной системы, как Kell-Cellano.

 

Клеточная линия, продуцирующая моноклональные анти-K-антитела, послу-жила основой промышленного производства тестовых реактивов для экспресс-определения фактора K и позволила устранить дефицит сывороток анти-K в лечебно-профилактических учреждениях Российской Федерации.

 

Моноклональные антитела со специфичностью анти-Kx получены Hattab и соавт. [Transfus. Med., 2003, V. 13, № 1, P. 43–48]. Антитела анти-Km не созданы.

 

Методы определения

 

70–80-е годы прошлого столетия определение фактора Kell среди больших контингентов лиц было весьма затруднительно главным образом из-за дефици-та типирующих реагентов. В настоящее время это исследование не представля-ет сложности ни в организационном, ни в методическом аспекте.

 

Для определения антигена K и антител используют метод конглютинации с 10 % желатином, метод агглютинации в солевой среде, непрямую пробу Кумбса [55], капиллярный метод [190], адсорбционные методы [3].

 

Выявление анти-K-антител также не представляет проблемы, однако следу-ет иметь в виду, что анти-K-антитела реагируют слабее в среде с низкой ионной силой (LISS) [282, 285, 382]. В частности, Merry и соавт. [282] установили, что

и этой среде значительно меньше молекул анти-K связываются с эритроцитами, чем в солевом растворе.

 

отдельных случаях анти-K-антитела не выявляются при использовании ав-томатизированных методов исследования [394], что, по-видимому, обусловлено их относительно низкой авидностью по сравнению с анти-D-антителами, к ко-торым, в основном, адаптированы существующие методы выявления антител.

 

последнее десятилетие созданы высокоактивные моноклональные реакти-вы IgM анти-Kell, позволяющие производить определение факторов Kell уско-ренным методом на плоскости.

 

Описаны методы определения генотипа K / k, основанные на полимеразной цепной реакции (ПЦР). Для этого используют участки рестрикции Bsm1 (BsaM1) аллеля K, но не k [232, 239, 294]. Используются аллель-специфические праймеры [94, 196, 232], гибридизация аллель-специфических олигонуклеотидов [283].

 

У беременных женщин K-фенотип плода определяют используя амниоциты. Молекулярно-генетические тесты не столь точны как серологические методы.

Отмечены ложноположительные результаты при исследовании лиц с фенотипом Ko.

При анализе необычных форм Kell-антигенов сочетание методов молекулярной диагностики с серологическими методами может оказаться более информативным.
